Consultation
All
patients are required to have an initial consultation
appointment.
This consultation appointment will
be to allow Dr. Naples to review your medical history
and make a complete evaluation of your needs.
At
your request, your referring dentist may send current
(within one year) xrays. If additional films are needed,
they may be taken at our office.
All patients
under the age of 18 must be accompanied by a parent or
guardian at the initial consultation and surgery
appointment.
For your convenience you may complete the
registration from and print prior to your scheduled
appointment. This will save you time at your
